The number of patients registered at the Orange County Free Clinic has risen from 86 at the end of 2007 to currently more than 3,500 today. New patients are being registered each week. The majority of patients seen by our nurse practitioner and physician live in households at or below 200% of the federal poverty level and are uninsured, more than 50% of our patients work in low -wage or part-time positions without medical insurance–with limited income to pay for medical care. The role of the Orange County Free Clinic as a health safety net, continues to be vitally important.
The two most diagnosed conditions at the Orange County Free Clinic are hypertension and diabetes. Patients coming to our offices suffer from multiple chronic diseases or conditions. A number of our chronically ill patients are on five or more mediations, compounding the seriousness and therefore requiring proper management for medication use. Only through our Chronic Care Management Program can we provide the level of service needed to improve outcomes, support wellness, and enhance quality of life for these patients. Careful attention to avoiding drug interactions, limiting unnecessary medications and providing the most appropriate treatment plan will lead to optimal outcomes for our patients.
